Our qualified nurses operate 24-hour support that aims to maximise the health and wellbeing of those in need. We will provide you with an individual assessment to determine which level of nursing care and support you need to meet both your physical and emotional needs.
Every resident has a named registered nurse and a team of carers ensuring continuity of care provided by familiar, friendly faces providing quality 24 hour nursing care.

Dementia Care / EMI Care

Oakfield Nursing Home features dedicated, compassionate, caring team members who are trained in helping our residents and their family members to live with both the good and bad days that go hand-in-hand with memory loss.

We follow the principles of person-centered care:

This approach aims to see the person with dementia as an individual, rather than focusing on their illness.

Person-centered care considers the whole person, taking into account each individual’s unique qualities, abilities, interests, preferences and needs.

We strive to ensure the dementia care we provide is appropriate to our residents needs and ensures their safety.

Support is provided to help maintain their independence and dignity.

Discharge To Assess / D2A

Discharge To Assess is a scheme that aims to reduce the time people spend in hospital, once they no longer need acute care, but need further time to recover and rehabilitate ready to return to their homes.

Oakfield Nursing Home works closely in conjunction with Morecambe Bay NHS Foundation Trust to ensure that all agencies work together with the aim to get people back to their homes as quickly and safely as possible.

Often prolonged stays in hospital are not appropriate and health outcomes are improved if someone can go home with an appropriate care package in place.

Our role is to make sure that people recover and recuperate in the best environment possible.
